 ancestors rank (if known)  (abbreviation please, example: pvt for private)

 ancestors name (format: John W. Doe or John Doe)

 company (A-K; can be looked up on our roll call page if unknown)

 descendants name to be listed

 relationship to 11th member (ie. great-grandson, great-great-granddaughter, great-great nephew)

 email address (www.11thtennessee.org will not share your email address with anyone other than visitors to this site)
